New Age Hospice is seeking a Clinical Manager – Hospice

The Hospice Clinical Manager is responsible for the day-to-day oversight of clinical services within the hospice program. This role ensures the delivery of high-quality, compassionate care that meets all regulatory, clinical, and organizational standards. The Clinical Manager leads and supports the interdisciplinary team, fosters a culture of excellence, and ensures compliance with all applicable laws and guidelines.
Essential Duties and Responsibilities:
- Provide leadership and supervision to clinical staff including RNs, LVNs, CNAs, social workers, chaplains, and other interdisciplinary team members.
- Ensure timely and appropriate patient admissions, care planning, and service delivery.
- Monitor and manage clinical performance metrics, including quality outcomes, patient satisfaction, and compliance indicators.
- Conduct regular IDT and education meetings to support care coordination and staff development.
- Oversee documentation practices to ensure accuracy, timeliness, and compliance with Medicare Conditions of Participation and state regulations.
- Participate in the hiring, training, evaluation, and professional development of clinical staff.
- Collaborate with the Administrator and other leadership to support census growth, operational efficiency, and strategic initiatives.
- Serve as a clinical resource and mentor to staff, promoting best practices in hospice and palliative care.
- Respond to patient and family concerns with empathy and professionalism, ensuring resolution and satisfaction.
- Participate in on-call rotation and provide clinical support as needed.
Qualifications:
- Registered Nurse (RN) required; Bachelor’s degree in Nursing or related field preferred.
- Current RN license in good standing TX.
- Minimum 1–3 years of hospice or palliative care experience, with at least 1 year in a supervisory or management role.

Working Conditions:
- Hybrid remote with regular field visits to patient homes, facilities, and team members.
- May require evening or weekend availability for urgent clinical needs or staff support.
- Participation in on-call rotation as needed.
