Position: Clinical Information Specialist - Remote
The Clinical Information Specialist will play a critical role ensuring that Monogram Health has the most up-to-date clinical data for our census and allow for accurate, evidence- based care management. He or she reports to the VP, Care Management Operations and will be charged with identifying and obtaining important medical information for Monogram members, including but not limited to ownership of outbound and inbound medical record requests, use of external clinical information resources and portals, as well as other priority initiatives related to optimization of care coordination and management through information.
Roles and Responsibilities
- Use reporting and analytics to prioritize work daily
- Manage outbound and inbound requests for a variety of medical records
- Update patient records with pertinent date received through records, such as lab values and communicate them to appropriate clinical personnel
- Perform outreach as needed to a variety of providers and vendors to captures and communicate important updates
- Ensure contact information for provider network contains accurate phone and fax numbers
- Participate in special projects and other duties as assigned
Position Requirements
- 2+ years working in the health care industry
- Top-notch customer service and interpersonal skills
- Ability to work independently and self-motivate
- Technology proficient with the ability to learn to use a variety of software reporting
- Ability to prioritize, learn, and adapt quickly in a fast- paced environment
