Responsibilities:
• The Clinical Social Worker will serve as an integral member of the HOPE Clinic and this position will provide case management support and will serve specifically as the HOPE Clinic liaison to the Department of Children and Family (DCF). The position also includes limited opportunity for psychotherapy/counseling, including individual, couples, or group.
• The Clinical Social Worker will also provide continuity for HOPE Clinic patients admitted to the inpatient setting by working closely with the Addiction Consult Team, inpatient obstetrics and newborn medicine teams.
• This position requires expertise in assessment, motivational intervention, psychoeducation, treatment and disposition planning, and case management/monitoring of patients with SUD and their families.
• The Clinical Social Worker will provide clinical services to patients/families that address psychosocial, environmental, age specific and cultural issues to maximize emotional, social and physical well-being and the effective use of health care and community resources.
• Position Reports To: Clinical Supervisor for Social Work.
Duties:
- Collaborate closely with a team of multi-disciplinary clinicians and community partners to develop and implement treatment plans for patients and families.
- Provide psychoeducation, motivational intervention, and cognitive behavioral therapy techniques for patients in an outpatient setting as indicated based on state of recovery and/or active substance use.
- Provide support and psychoeducation around attachment and parenting skills.
- Participate in and support multi-disciplinary team in safety assessments for families.
- Support patient and team in crisis stabilization.
- In collaboration with resource specialist, RN care manager and other team members provide case management for patients and connect them with appropriate community resources.
- Provide continuity for patients who are admitted to the hospital or other MGB services by communicating with the patient and the care team in the inpatient setting during the admission.
- Serve as a liaison to the MGH Child Protection Team and the Department of Children and Families (DCF) for the HOPE Clinic team.
- Work with resource specialist and RN care manager to help ensure and coordinate safe and successful transitions in care.
- Create a Plan of Safe Care/Family Care Plan together with patients prior to their delivery and updated safety planning as indicated.
- Create and maintain collaborative relationships with community partners including residential, intensive outpatient, DCF, early intervention to support patients.
- Potential for supervision of SW interns.
- Other duties as assigned.
Qualifications
Qualifications
Master's of Social Work (MSW) from an accredited program required.
Require current Massachusetts Licensed Independent Clinical Social Worker (LICSW). Candidates with Licensed Clinical Social Worker (LCSW) credential will be considered based on experience but must obtain LICSW within two years of hire.
Experience working with people with SUD and their families preferred.
Strong assessment and crisis management skills.
Knowledge of community services and resources.
Strong therapy skills, including Motivational Interviewing, Addiction therapy, Cognitive-Behavioral and other behaviorally oriented therapy modalities.
Excellent organizational skills.
Strong collaborative, customer service and advocacy skills.
Demonstrated ability to communicate effectively orally and in writing.
Strong interpersonal skills enabling effective team collaboration.
Demonstrated ability to understand the role of social worker in a hospital setting and to work independently in a complex, fast-paced environment.
Skills/Abilities/Competencies
Knowledge, skills and ability to assess patient/family needs for services and develop treatment plans that effectively address those needs.
Ability to maintain effective working relationships with patients/families and staff.
