Pharmacy Technician - Medication Reconciliation Specialist
Remote | Full-Time | Schedule: 6 AM-2:30 PM or 9 AM-6 PM PST
Pay Range: $18-$22/hour
(Dependent on licensure, certification, and geographic location)

What You'll Do
Medication Reconciliation:
- Verify patient medications, clarify dosages, reconcile discrepancies, and share accurate medication lists with healthcare providers.
Clinical Issue Identification:
- Spot potential medication-related issues (e.g., duplications, interactions, or dosing errors) and escalate them to clinical pharmacists for resolution.
Patient Education:
- Explain clinical program benefits, provide medication guidance, and support patients in understanding their treatment plans.
Collaborate with Providers:
- Work with providers to ensure accurate medication regimens and seamless delivery of clinical interventions during care transitions.
Accurate Record Keeping:
- Maintain thorough, accurate documentation in the software platform for all patient interactions and clinical details.
Follow-up & Coordination:
- Liaise with hospitals, clinics, pharmacies, and patients to address concerns and ensure timely care coordination post-discharge.
Schedule Clinical Consultations:
- Coordinate telephonic or video appointments between patients and clinical pharmacists for enhanced support.
Process Improvement:
- Participate in team meetings to streamline workflows and improve service delivery.
Additional Responsibilities:
- Assist with obtaining referrals and authorizations.
- Participate in projects to support team goals.
Required Qualifications
- Licensure: State Licensed Pharmacy Technician.
- Certification: National Pharmacy Technician Certification (PTCB or ExCPT).
- Medication Knowledge: Familiarity with medical terminology and brand/generic medication names.
- Communication Skills: Excellent written and oral communication skills.
- Computer Proficiency: Strong skills in Microsoft 365, Google Workspace, and MacBooks.
- Self-Motivation: Ability to prioritize tasks and work independently in a fast-paced remote setting.
- Schedule: Commitment to a 40-hour work week.
Preferred Qualifications
- Experience with electronic medical records (EMRs) or healthcare platforms.
- Ambulatory care experience, particularly in medication history and reconciliation.
- High-volume healthcare environment experience.
- Bilingual proficiency (a plus).
