Position Summary:
The Chronic Care Management Enrollment Specialist at Recora is responsible for enrolling patients in Recora’s care management program. This role involves working closely with healthcare providers, patients, and their families to ensure effective communication and enrollment in chronic care management. The Enrollment Specialist will focus on enhancing patient engagement, patient outreach, and achieving program enrollment targets.
Key Responsibilities:
- Initiate outbound calls to eligible patients to introduce and explain the CCM program
- Educate patients on the benefits of CCM, including monthly check-ins, care plans, and access to 24/7 nurse support
- Verify patient identity and eligibility for the CCM program
- Clearly communicate program details, including Medicare coverage and any applicable copays or deductibles
- Address patient questions and concerns about the CCM program
- Obtain and document patient consent for enrollment
- Manage a queue of eligible patients and prioritize enrollment efforts
- Maintain accurate records of patient interactions and enrollment status
- Collaborate with healthcare providers and care managers to ensure smooth patient onboarding
- Stay up-to-date on CCM program guidelines and best practices
