Currently, we are looking for a Licensed Vocational Nurse (LVN) to join our Community Health team as part of the CalAIM ECM Program. In this dynamic role, you will play a crucial part in delivering essential healthcare services to underserved populations, ensuring that every individual receives the attention and support they deserve. Expected Hours: Monday through Friday 8:30AM to 5:00pm (40 hours per week)
Work Location: Hybrid remote in Santa Clara County, California
Responsibilities
- Provide direct nursing care to clients in community settings
- Assist in the assessment and evaluation of clients' health needs
- Develop and implement individualized care plans in collaboration with the healthcare team
- Monitor and document clients' health status and progress
- Educate clients and their families on healthcare, wellness, and preventive measures
- Coordinate with multidisciplinary teams to ensure a holistic approach to care
- Support clients in accessing mental health and social support services

- MUST reside in Santa Clara
- Current and valid LVN license in the state of practice
- Minimum of 1 year experience in a healthcare or community health setting
- Strong understanding of mental health care principles and practices

- Possess a valid California Driver’s License (Class C minimum), maintain a personal, operable vehicle for daily business use, and carry current liability insurance that meets California's minimum legal requirements.
