$24.00 To $26.00 Hourly
Are you a bilingual (English/Spanish) LPN, LVN passionate about providing proactive patient care? Nsight Health, Inc. is seeking a Remote RPM to manage patient alerts and conduct outreach to improve patient outcomes. If you have a strong background in patient education and engagement, we want you on our team.
Shift Differential Pay:
- $1.50 per hour for all hours worked after 7:00 PM EST
- $1.50 per hour for all hours worked on Saturdays and Sundays

Responsibilities:
- Manage a high volume of outbound calls to check on patients and address health concerns
- Monitor and respond to RPM alerts, ensuring timely follow-up on acute and chronic conditions
- Communicate with healthcare providers to escalate concerns and coordinate patient care
- Conduct monthly wellness assessments and perform chart reviews to track patient progress
- Accurately document all interactions in the CCM/RPM software
- Work towards personal and team performance metrics to improve patient engagement
- Foster a collaborative and positive work environment in line with Nsight’s culture of compassion and excellence
Qualifications:
- Active LPN/LVN license (Preferred)
- Bilingual – Fluent in English and Spanish (Required)
- Passionate about patient care and education, with a strong ability to connect with patients with empathy
- Moderate to advanced computer skills for documentation and communication
- Reliable, high-speed internet and a HIPAA-compliant home workspace (Speed test required during the interview)
