DescriptionSummary:
Responsible for coordinating clinical quality management and performance improvement activities related to assigned functions such as peer review, core measures, outcome reporting or internal auditing. Provides leadership and support for clinical quality initiatives; serves as liaison to other medical staff and quality committees.
Responsibilities:
- Develops implements and maintains policies and procedures for programs to ensure quality patient care and services
- Monitoring and tracking patient outcomes through advanced techniques
- Ensuring adherence to quality standards and reviewing program deliverables
- Assists in the interpretation of policies and objectives of nursing service to staff and community groups
- Creating care plans, schedules, estimates, resource plans and budgets
- Conducting home care program meetings to ensure effective management of activities
- Incorporates patient safety practices/guidelines to promote a safe environment resulting in positive patient outcomes
- Demonstrates accountability for nursing research and quality improvement activities
- Provides evidence-based nursing care
- Communicates patient information effectively across the continuum of care
- Other duties include, but are not limited to assisting with the developing and meeting annual goals and objectives for the department, building strategic relationships within and across departments, and developing and maintaining a comprehensive interdisciplinary performance improvement and patient safety program
Requirements:
