The Medical Director for the House Calls Program leads interdisciplinary in-home primary care teams focused on supporting high-need patients. Responsible for team management, oversight, and development of care programs, this role involves direct patient care and collaboration in clinical model feedback. - The Medical Director will be responsible for leading, as adyad partner with the Mobile Practice Manager, the in-home primary care provider teams in the specified geography.
- The At Home division is an interdisciplinary team that focuses on primary care and transitional care support for our highest-need patients in their homes.
- The primary responsibility for the Medical Director is direct management, oversight, and coaching of the local team, and is part of the At Home divisions leadership team which helps develop programs (i.e., complex care coordination, chronic disease management, psychosocial support, etc.) throughout the division.
- The Medical Director will see patients in their homes up to 3days aweek alongside their provider colleagues. Additionally, we see this leader being akey thought partner in providing feedback on our clinical model especially as it pertains to the At Home Program.
