Job Details:
- Board-certified in Internal or Family Medicine
- 4+ years of experience managing Medicare, Medicaid, and commercial populations
- M-F 8AM-5 PM
- Oversee 2 clinics
- Will start out at 50% clinical and 50% admin and then switch over to 70% administrative and 30% clinical
- Ability to treat the acutely ill in an outpatient setting offering hydration, IV, antibioitcs, wound care, and other services
- Chronic illnesses with substance abuse Candidate must be OK with prescribing opioids
- Complex care, Incoming patients are invite-only
- Looking for someone who has experience treating patients with multiple chronic diseases and truly believes in the value of a team-oriented patient-centered medical home model
- EMR is E clinical works, Onsite infusion center
- BLS certification and a strong focus on customer service is required
- Experience in population health management and working with psychosocially and medically complex patients is strongly preferred
