About: Logistics Health Incorporated (LHI) is in need of a Ophthalmologist to perform Disability Benefit Questionnaires with patients who are Active Duty or Veterans of our Armed Forces. This provider will be performing General Medicine DBQ’s and submitting their findings and the DBQ form through the VA portal.
These exams are for Pension and Benefits evaluations done by the VA for a veteran. This will determine the amount of compensation and health benefits given by the VA to the veteran for their Disability which resulted in their service.
Parameters: They will consider an Ophthalmologist DBQs. These providers must be board certified.
Will client consider candidate needing license? Needs an active License in any state.
Job/Assignment Details:
Will spend 45min to hour per patient.

Provider will need laptop to connect to their Web-based documentation system.
All needed instruments that will be onsite.
No Treatment required, one time evaluation
Reason for Coverage: Shortage of providers available to perform these exams
Setting: (inpatient/outpatient/mix) Outpatient
Start Date: Immediately, once credentialed this provider will be put in the pool to be called upon to perform these exams. The exams come from the VA to LHI and LHI then will ask for the upcoming availability of our provider in order to schedule this exam.

Credentialing timeframe: Credentialing takes approximately 2 weeks. The provider will then be required to take 4 hours of online trainings so they are up to speed on how to perform the exam and submit their findings online to the VA.

EMR: Web based portal used to fill out the DBQ and submitted to the VA. Provider will have access to the patient’s previous medical records 48 hours in advance of the appointment in order to prepare appropriately. Our provider is required to submit the DBQ to the VA through the portal within 48 hours of finishing the onsite exam.
Rates/Travel
Pay: $170/hr
Full Day Bill Rate - $1,750 (These are recommended rates)
Half Day Bill Rate - $875 (These are recommended rates)(They are willing to consider higher rate)
NO Travel since this is an embedded site, they would prefer someone local or drive-in

No name clear required. I will present to my main contract for review and will have a PIV set up within 72 hours of submission.

Facility Info: The sites where these exams are performed are local clinics that contract with LHI to provide a patient room for our providers. In most cases there is an MA onsite to help the provider with checking in the patient and assisting the provider with their needs.
