Your Role as MDS Director As the
RN/LPN MDS Director, you will be a cornerstone in managing and coordinating resident assessments, care planning, and interdisciplinary teamwork. You will ensure compliance with federal and state regulations while supporting a high standard of care.
Key Responsibilities Include: - Overseeing the development and completion of MDS assessments and care plans to meet regulatory requirements.
- Ensuring timely submission of assessments to the State repository per RAI Manual guidelines.
- Collaborating with the Interdisciplinary Care Team to tailor care plans to each resident's unique needs.
- Monitoring Quality Measures Reports to enhance outcomes and support our Five Star Rating.
- Providing education and support to residents, families, and care staff about care planning.
- Participating in facility surveys and government inspections, ensuring compliance and readiness.
