We are seeking a dedicated and compassionate Physical Therapy Assistant (PTA) for PRN to join our team of skilled home health care providers. The ideal candidate will have a strong commitment to providing quality care. Working under the supervision of a licensed Physical Therapist, you will provide physical therapy services to patients in their own homes, delivering therapeutic exercises, and providing patient education to ensure effective and safe care in a home environment.

Job Description Summary:
- Assists in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy
- Treats clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, light and electricity
- Teaches, supervises, and counsels the family and client in the physical therapy program
- Instructs client in care and use of adaptive devices such as: wheelchairs, braces, crutches, canes and prosthetic and orthotic devices
- Arranges for out-patient service for procedures when appropriate
- Records on a timely basis all treatments, and client's response to therapy intervention Also records quality assurance data
- Communicates with PT and other team members (including Home Health Aides), when appropriate. Instructs them in certain phases of physical therapy that they may use while working with the client
- Under the direction of a Physical Therapist, writes clinical/progress notes on the date of the visit and incorporates same in the client's record according to company policy
- Participates in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary.
- Participates in effective coordination of schedule to ensure all reassessments are performed in a timely manner
- Obtains a medical history, particularly as it relates to the present condition
- Identifies possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and reports same to the PT
- Observes, and records activities and findings in the clinical record and reports to the PT and the physician any adverse reaction to treatment
- Participates with staff, clients, and physicians in discharge activities and assists the Physical Therapist in completing the Physical Therapy Discharge Summary

Position Qualifications
· Must be licensed by the State as a Physical Therapist Assistant
· Must be a graduate of an approved school giving a course of not less than two years for Physical Therapist Assistants
·Must have one year of experience in an acute/sub-acute setting
·The ability to drive and the ability to access clients’ homes, which may not be routinely wheelchair accessible are required
·Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care
·May be required to bend, stoop, reach and move client weight up to 250 pounds, lift and/or carry up to 30 pounds
