Summary
Responsible for the delivery and documentation of care delivered to Pulmonary Rehab Phase II and Phase III outpatients, as appropriate to services provided at specific location, through identifying patient needs, diagnoses, planning, implementation and evaluation. Responsible for directing and coordinating care for Pulmonary Rehab patients based on established Pulmonary Rehab practice standards subscribed to by the American Association of Cardiovascular and Pulmonary Rehabilitation. Demonstrates proficiency in Pulmonary Rehab core competencies. Possesses knowledge of Pulmonary Rehab guidelines, standards and reimbursement. Collaborates with other professional disciplines to ensure effective and efficient care delivery and the achievement of desired patient outcomes. Supports the organizations vision and mission. Uses knowledge of patients age and cultural diversity in the provision of care. Contributes to the provision of quality care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in patient care. Contributes to the provision of community outreach through participation in system, division, and department programs and events.
Requirements
Education: Bachelors Degree in Exercise Physiology required.
Experience:Pulmonary Rehab clinical practice (12 weeks) required; 1-3 years experience preferred.
Certification: BLS (American Heart Association/Basic Life Support) required. ACLS (Advanced Cardiac Life Support) preferred. ACSM Exercise Specialist certification and/or Registered Exercise Physiologist. In lieu of certification, 3-5 years experience in Cardiac Rehab will be considered.
License: N/A
Other: Must be a team player and have good critical thinking, organization, communication, teaching, interpersonal, customer service, analytical and computer skills.
