POSITION SUMMARY Assess & evaluate the physical and psychological wellbeing of the cardiac rehab patient, monitor the exercise plan and help the pt. to achieve progress towards mutual goals of risk factor identification and modification to long term plan and assessment during rehabilitation. Participate in data collection for pt. outcomes & implement and insure patient satisfaction, exhibit exercise leadership skills and teach patients/families/significant others to expand knowledge of recovery and wellness.
To coordinate, implement, and plan nursing care in a timely and accurate manner.
PERFORMANCE STANDARDS Formal Education / Training:
- Licensed by the State of Tennessee or Compact State.
- Earned a nursing diploma or degree from a college or university.
- Proficient in EKG interpretation.
- BLS within in 90 days of hire.
- ACLS within 6 months of hire.
Workplace Experience:
Patient Care Delivery: Prefer experience in out-patient cardiac rehab with the ability to educate patients and families on risk factor modification for heart disease.
Equipment and Skills Training:
The ability to work well under pressure, to function dependently and independently and to be flexible in applying basic nursing knowledge to a variety of setting. Equipment: Cardiac Monitor, Treadmills, Exercise Bicycles, Rolling Machines, Stairmaster, Exercise Weight Machines/weights.
Physical Environment:
A cardiac rehab unit with patient population ranging from less than one year of age> ninety-nine years of age.
Physical Effort:
- Requires the ability to communicate in English orally and in writing.
- Requires prolonged standing and working for up to twelve hours a day.
- Constantly required to push/pull objects up to 175 lbs.
- Requires the ability to lift and turn patients with assistance up to 250 lbs.
PERFORMANCE STANDARDS - Performs initial and ongoing assessment of patients' conditions and needs by selecting, administering, and interpreting results of evaluation processes
- Develops plan of care based on patients' history and medical diagnoses.
- Conducts and implements exercise prescriptions according to AACVPR and Williamson Health guidelines.
- Conducts group exercise classes according to AACVPR and WMC standards.
- Provides appropriate education/consultation to patients/significant others.
- Administers patient medications and monitors effects of patient medications as needed.
- Completes timely and accurate documentation.
- Provides regular patient reports to referring physicians and Medical Director.
- Monitors and maintains department equipment including but not limited to exercise equipment, telemetry monitors, oxygen saturation monitors, and LSI documentation system.
- Contributes to department's success by participation in improving processes, tracking and reporting patient outcomes, and actively participating in processes to promote AACVPR accreditation.
