The Exercise Physiologist monitors outpatient cardiac rehab and may perform a variety of non-invasive cardiovascular exams.
- Provides care including the development, implementation and evaluation of exercise programs for cardiac patients in phase 2 and 3 of cardiac rehabilitation.
- Works collaboratively with all members of the patient's care team to ensure services are consistent with post op and/or post hospitalization care.
- Evaluates and revises plan of care as needed in collaboration with RN and physician when necessary.
- Develops and advances the patient's exercise program based on the physiological and electrocardiographic response to exercise.
- Provides health education and teaching for patient and family.
- Complete required feedback and communication to patient and healthcare team including EMR documentation and reports concerning education, exercise progression, exercise treatment and monitored physiological and vital responses of the patient.
- Assists with scheduling and billing of cardiac rehab services.
- Precepts nursing, exercise physiology and nutrition students as assigned to the program.
- Schedules patient for cardiac rehab initial visits
- Conducts initial visit patient appointments
- Performs patient heart disease risk factor assessments
- Coordinates patient care plans for outpatient cardiac rehab, phase II and phase III
- Coordinates patient exercise class schedules
- Monitors patients on telemetry during exercise sessions
- Leads patients through exercise prescriptions and assists patient with exercise equipment
- Provides education to patients to support risk factor modification
- Reports adverse events to ordering providers
- Monitors patients during exercise stress tests
- Applies ambulatory holter and event monitors
