****Average hourly rate is $17.85 (including wage, incentives, bonuses, overtime, shift differential, etc.)****
POSITION PURPOSE
Supports older youth in the Permanency Services Program by coordinating with staff and agencies, supervising the Youth Leadership Council, and assisting youth in independent living preparation.
RESPONSIBILITIES
- Youth Support: Assists with Child Without Placement cases and facilitates Life Skills classes.
- Independent Living: Coordinates independent living services and assesses eligibility for programs like Extended Care and Return to Care.
- Youth Leadership Council: Manages council activities, needs, and projects.
- Documentation: Maintains thorough records for the Preparation for Adult Living program and completes assessments, including the Ansell Casey Life Skills Assessment.
- Event Coordination: Assists in planning and supervising statewide/regional teen events and camps.
- Data Reporting: Completes National Youth in Transition Database (NYTD) entries and submits reports as required.
EDUCATION, EXPERIENCE, AND QUALIFICATIONS
- Required: High School Diploma or GED with 2 years of experience in social services or working with at-risk populations.
- Skills: Strong communication, organization, and documentation abilities; cultural sensitivity; basic computer skills.
- Other: Valid driver’s license with ability to drive for agency business. Lived-Foster Care Experience Preferred
PRINCIPAL ACCOUNTABILITIES
