****Average hourly rate is $30.03 (including wage, incentives, bonuses, overtime, shift differential, etc.)****
POSITION PURPOSE
Enhances placement stability for children in foster care by facilitating PS-TDM meetings with children, families, professionals, and natural supports, working towards consensus on placement decisions.
RESPONSIBILITIES
- Meeting Facilitation: Conduct structured, outcome-focused PS-TDM meetings with family members, foster parents, caregivers, agency staff, and service providers to improve placement stability.
- Process Guidance: Lead participants through a six-stage process to discuss concerns, identify strengths, and develop consensus-based placement solutions.
- Safety Assessment: Ensure participant safety before, during, and after meetings.
- Documentation & Reporting: Accurately document meeting content, maintain timely records in the PS-TDM data application, and provide feedback on service gaps and policy issues.
- Training & Support: Educate attendees on the PS-TDM process and provide support for productive, respectful interactions.
- Compliance & Advocacy: Conduct meetings in alignment with agency values, promote cultural sensitivity, and advocate for system improvements.
- Transportation: Drive agency or personal vehicles when required for client or agency purposes (valid driver’s license required).
SUPERVISION
No direct supervisory responsibilities.
EDUCATION, EXPERIENCE, AND QUALIFICATIONS
- Education: Bachelor’s degree in human services or a related field.
- Experience: Minimum of two years in child welfare services (intake, investigation, or ongoing family services).
- Skills: Strong communication and analytical skills; ability to work with financial and technical data; cultural sensitivity.
PRINCIPAL ACCOUNTABILITIES
- Facilitate effective PS-TDM meetings to improve placement stability.
- Guide participants through structured decision-making processes.
- Document and report meeting outcomes and trends to support continuous improvement.
