The Oncology Nurse Navigator (ONN) uses their oncology-specific clinical knowledge to offer individualized guidance to patients, families and caregivers to foster independence during their complex cancer journey. Using the nursing process, an ONN assesses patient needs and provides education and resources to facilitate appropriate and timely quality health and psychosocial care throughout all phases of the cancer continuum. (ONS definition). The ONN creates an environment of care that is patient-centered.
Hours: 40 hours/week
Responsibilities include:
Participates in day-to-day operations of the program including patient education and planning of chemotherapy treatment.
Serve as a liaison and advocate between the patient and family, primary care physician, internal and external care providers, specialists, support network members and the healthcare community.
Effectively and efficiently plans, coordinates, implements and monitors care across service lines for patients requiring oncologic care.
Identifies and provides guidance to patients and eliminates potential barriers to care by identifying and referring to appropriate resources.
Communicates and collaborates effectively with diverse audiences including professional staff, patients, community and other service lines to promote and provide comprehensive, quality care.
Builds relationships with patients and physicians to coordinate a plan of care to include education, appointments and transportation.
Participates in program evaluation and development of strategic goals consistent with the Cancer Center mission and professional values.
Participates in the QA/QI projects for the program.
Supports the research activities of the program.
Exhibits professional commitment through individual integrity, honesty and accountability to self and others
Delivers expert clinical care through evidenced-based practice partnerships with patients, families and colleagues.
