Assume the responsibilities for direct care of assigned patients under the supervision of a registered nurse or provider in patient care departments. Provides nursing services to patient and families in accordance with the LPN scope of practice as defined by the Illinois Board of Nursing.
- Provide direct care to patients under the direct supervision of an RN, mid-level provider or physician.
- Patient response to care is reported to a registered nurse/provider for evaluation, intervention and modification of the plan of care.
- Ensures documentation of care is complete and accurate.
- Takes responsibility for patient and employee safety by reporting or/and correcting safety hazards
- Performs all responsibilities and duties required by unit as specific to the scope of service
- Ability to provide patients medications utilizing the five rights.
- Routine patient care involving hygiene care-including routine rounding of patients
- Documentation of care
- Receives bedside report from offgoing nurse at the beginning of each shift, and updates communication board
- Effectively communicates within the health care team to promote interdisciplinary collaboration
