ResponsibilitiesThe Travel Nurse participates in admissions, assessment, evaluations, and documentation to ensure that patients are receiving exceptional care. As a Travel Nurse you will:
- Under the physician’s order, admit patients eligible for hospice services
- Assess and evaluates patient needs/problems as outlined in the patient IDG Plan of Care; identify mutually agreed upon goals with patients and families
- Document daily changes and updates to the patient care plans, including discharge planning, and notify attending and hospice physician through scheduled IDG meetings
- Participate in Quality Assessment and Performance Improvement (QAPI) program through submission of data collection relating to patient care problems
- Attend skilled nurse in-services and meetings, and interdisciplinary team meetings
- Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Qualifications- Current unencumbered Registered Nurse (RN) state license or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
- Three years of experience as a registered nurse in a licensed hospice or home health setting including team supervision
- Current driver’s license and automobile in good working condition with proof of auto insurance
- Demonstrates above average computer and EHR competency
- Able to handle multiple and shifting assignments
- Ability to make sound professional clinical judgment in a field setting
Posted Min Pay Rate USD $85,651.00/Yr.
Posted Max Pay Rate USD $104,468.00/Yr.
Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
