The brokerage supports customers that live in Multnomah and Clackamas County, and they reflect the diverse fabric of these communities. People experience disability across class, race, gender, and ethnic identities. Some of the people we serve live with their families or friends, others experience homelessness, and others live in their own homes and apartments. We work with people who experience a wide array of disabilities and some may also experience mental health conditions and addictions, or be youth transitioning into adulthood.
UCP Mentors assists customers to navigate the support services system, including using person-centered planning techniques to develop plans and goals; securing resources and services (in areas such as employment, housing, crisis, and direct support providers); and addressing their health and safety needs.
The AQC evaluates and oversees the quality of services that customers are receiving from their designated Personal Agents. They conduct the Oregon Needs Assessment that is used to set service hours and rates for the customers that we support. The AQC will employ strengths-based evaluation techniques when implementing the Oregon Needs Assessment.
The AQC will facilitate the assessment from beginning to end, work collaboratively with the customer’s Personal Agent, address discrepancies, resolve complaints, and complete corresponding documentation. The AQC will act as the primary liaison with customers, families, and other important people in the customer’s life to inform them about the purpose and results of the assessment.
In addition, the AQC will implement quality assurance measures while fostering a culture of transparency and accountability in partnership with the brokerage team. These measures will be informed by the customers, an Advisory Board, employees, and the leadership team. Quality improvement measures will be trauma-informed and incorporate equity practices to improve the overall quality of services and ensure compliance with the regulatory entities.
The ideal candidate will be a skilled facilitator adept at drawing out necessary information while honoring the dignity of the customer and their personal life. The candidate will be able to establish rapport quickly with customers from diverse backgrounds and accurately identify their strengths and needs within a limited time-frame. The successful candidate will also exhibit high-level communication and collaboration skills with the brokerage team.

JOB DETAILS:Hours & Location:
- This is a 40-hour a week job. Typical schedule will be Monday through Friday, 8:30am to 5pm but may vary depending on the customers’ needs. Additional hours will rarely be required.
- This position is eligible for the hybrid model of office and remote work. However, during your training period, you will be required to come into our office located at NE 102nd Avenue.
- This position will require you to drive to meetings all over Multnomah and Clackamas County.

- Bachelor’s degree in social services or closely related field and at least 2 years of work experience in developmental disabilities, social services, mental health or a related field; or an Associate’s degree in social services or closely related field and at least 3 years of work experience in developmental disabilities, social services, mental health or a related field; or a high school diploma/GED and at least 5 years of work experience in developmental disabilities, social services, mental health or related field.
- Must be able to drive on the job, which means that you are at least 18 years old, have a current driver’s license, a reliable personal vehicle, at least two years of driving experience, auto insurance, and a clean driving record.
- Must be able to pass a reference check and criminal history background check.
- Demonstrated commitment to customer self-determination and personalized support.
- Demonstrated ability to prioritize, meet deadlines, and manage multiple projects in a fast-paced environment.
- Demonstrated ability to identify and resolve problems in a timely manner.

- Preferred qualification is fluency in a second language including ASL.
- $30.97 per hour. Annual wage increase based on satisfactory performance.
- Eligible for overtime pay for hours worked over 40 hours per week.
