Community Health Worker-Patient Navigation: Applications are accepted on a rolling basis.

Location: Hybrid (In-person hours at Jamaica in Queens, NYC)
Hours per week: 21 hours per week for clinical work. Interns will receive training during their first week to prepare for their role as navigators. The internship will be paid if the candidate is available for and successfully completes an additional 6-9 hours of coursework at Hostos Community College each week. We will prioritize applicants who can serve until December 2025.
We are looking for CHW Navigators to perform on-site and telephone-based patient navigation for our Strong Children Wellness Medical Group practices in Jamaica, Queens.
Interns should be interested in working with marginalized populations, including those affected by poverty, immigration challenges, homelessness and/or child welfare system. These interns will be responsible for:
Screening, navigation and referral to community resources for unmet social needs; Arrangement of transportation services and subspecialty appointments for patients; Telephone support and text follow-up for patient concerns Data entry of patient screening questionnaires into electronic platform. Depending on navigator experience, managing a small panel of patients receiving care management
Our patient navigators will provide mostly remote and in-person support as needed. They must be able to attend weekly meetings with the lead navigator to report progress throughout the week. Data organization includes working with our EMR and our HIPAA-compliant text messaging service, OhMD, for which training will be provided.
Patient Navigators can earn course credit for their work. As desired, there are also additional research activities outside of clinical work, including data collection to support manuscript development and conference presentations
Qualifications:
Must be available for at least 21 hours per week for hybrid clinical work duties plus an additional 6-9 hours per week of CHW coursework at Hostos Community college to be considered for paid internship Must be able to attend virtual community health worker coursework on Tuesdays and Thursdays from 1-4pm (February- May 2025) Have excellent communication skills and attention to detail, demonstrate flexibility, and possess strong teamwork skills Experience working with vulnerable families and diverse communities (e.g., low-income, immigrants, et, preferred but not required
Preferences:
Have Spanish language fluency ( Full professional proficiency or native fluency) Six or more months of full-time relevant experience with any of the following: individual case management, mentoring, or group work; providing support to individuals or groups to address specific needs.; community health or outreach activities; supporting health and wellness initiatives and public education within local communities; or engagement with culturally and linguistically diverse communities and with historically marginalized groups. Eligible experience includes work with government agencies or community organizations that provide public services such as: Programs aimed at assisting community members in accessing resources and supportive services. Initiatives that maintain partnerships with schools, community organizations, or other government agencies to promote health and wellness programs. Examples of an applicant's relevant experience may include: • Coursework in public health or community health, a plus. Able to work until December 2025.
