Position Summary:
The Clinical Leader – RN Case Manager assesses the needs of the department and collaborates with health team members to coordinate and complete timely assessment, planning, implementation and evaluation of discharge plans and care transitions across the continuum of care. Under the supervision of the Clinical Manager/Director, this position supports the RN Case Managers in the department according to policies and procedures.
Assists with the daily operational aspects of the department with respect to facilitation of work priorities.
Maintains professional educational development.
Guides the professional growth and development of RN Case Managers.
Participates in quality improvement activities.
Regularly perceives potential problem situations and intervenes to offset adverse impact.
Ensures departments compliance with regulatory standards.
Acts as a resource to staff
Identifies and communicates special staffing needs and/or problems to the Clinical Manager/Director.
Hours: 40 hours/week, Day Shift
Rotating weekends and holidays
Location: Main Campus, 295 Varnum Ave Lowell, MA
Qualifications:
Massachusetts RN Licensure.
3 Years of RN Experience with at least 1 year as a charge/lead RN.
Current Basic Life Support (BLS) Certification.
Bachelor of Science in Nursing (BSN).
MSN preferred
