JOB PURPOSE:
The Pulmonary Navigator coordinates and manages outcomes of a specific patient population to facilitate achievement of quality, services, and cost. Provides one to one education, telephonic disease management services, and home visits to individuals with pulmonary diseases. Assigned to patient caseload from initial inpatient admission ensuring a smooth transitioning of care to post-acute care. Works collaboratively and maintains active communication with physicians, Care Coordinators and interdisciplinary staff. Prepares, analyzes, and communicates profiles, reports, and summaries. Identifies and evaluates patient and family educational needs.

Qualifications:
MINIMUM EDUCATION REQUIRED:
Graduate of an accredited school of Respiratory Therapy; Bachelors degree strongly preferred.
MINIMUM EXPERIENCE REQUIRED:
Three years of recent and relevant clinical experience related to respiratory care. Prefer experience with multidisciplinary teams. Experience with data analysis, Centers for Medicare/Medicaid Services, research, and process improvement.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW: Current respiratory care practitioner licensed by the State of Georgia under the Composite State Board of Medical Examiners, hold Registered Respiratory Therapist (RRT), credential from National Board for Respiratory Care (NBRC). Credentialed as Pulmonary Disease Educator via American Association for Respiratory Care (AARC) within 6 months of hire. Basic Life Support (BLS) certification.
ADDITIONAL PREFERRED QUALIFICATIONS:
Advanced Cardiopulmonary Life Support (ACLS) preferred. Strong working knowledge of healthcare systems and community resources. Able to establish strong working relationships within a healthcare system.
