Job Overview:
The Transplant Patient Navigator will support transplant nurse coordinators and physicians with patient care management as it relates to pre-transplant recipient and donors and the transplant program. Coordinates patient care under the direction of the clinical team, including but not limited to coordinating patient testing and scheduling; reviewing clinical correspondence (i.e. lab results) for appropriate action; transplant database entry; monitoring patient lists for compliance with monthly blood specimens; obtaining patient medical records and assisting the RN or other team members with patient care issues such as blood draw or vital signs. Develops relationships with referring physicians, consultants, affiliates and office staff.
Qualifications:
Required:
- One to three years of related experience within the healthcare field
- Ability and interest to learn clinical information and criteria
- Knowledge of Medical Terminology
- Emphasis on computer literacy and communication skills, such as Microsoft Excel and Word
- Successful completion of all orientation programs
Preferred:
- Associate or Bachelor s degree in health related field
- Experience as a Patient Navigator
Scheduling Requirements:
- Shift Working: Monday-Friday, 8:00am-4:00pm
- Day Shift
- Full-Time, 37.5 Hours Per Week
Essential Functions:
- Prepare patient charts; request necessary clinical information from dialysis units and/or MDs; prepare charts for revisits/re-evaluations by ensuring all tests ordered are retrieved and ready for review; add missing forms to chart as needed such as progress notes, medication records; label each page with appropriate identifiers
- Obtain pending lab and/or test results at the direction of the RN coordinator. Findings then reported to RN/MD and initials of RN/MD secured on the report upon review. May enter results of tests into medical record
- Schedules outpatient testing and consultations upon request of RN coordinator. Enters corresponding action into medical record
- Assists clinical staff with specific patient care needs (follow-up phone calls, coordinating testing, scheduling tests/procedures, securing insurance authorization /pre-certifications, etc); may monitor patient lists for compliance, such as monthly specimens
- Assumes responsibility for order and integrity of patient's medical records
- Monitors medical records for regulatory compliance with requirements such as presence of patient identifiers; use of unapproved abbreviations; updating patient care summaries etc.
