The outpatient Cancer and Blood Disorders Clinic in SJCH is seeking a nurse coordinator to provide individualized, goal-directed patient care to patients and families at the expert level utilizing the principles and practices of the nursing process. We are looking for an individual who is motivated to build relationships with both team members and patients to enhance clinic efficiency, education, and community outreach to improve our patients' outcomes.
The pediatric hematology oncology RN coordinator will participate as an expert practitioner in a variety of direct and indirect patient care activities. RN coordinator will work with multidisciplinary team to deliver quality and safety of care through the continuum of care from the patient’s first clinic visit and throughout the continuation of their care. This position will directly report to the Ambulatory Service Manager for the outpatient Cancer and Blood Disorders Clinic at SJCH.
Schedule: Monday-Friday 8-4:30pm, no weekend and no holidays.
Orientation: Typical training range is 6 weeks to 6 months, based on experience and previous knowledge.
Potential schedule post training: ability to incorporate hybrid or remote work within the Monday-Friday schedule.

Job Summary/Purpose: Job duties include utilizing the nursing process (Assessment, Diagnosis, Planning, Implementation & Evaluation) in coordinating care for patients in the clinic setting. This includes order entry, patient message and phone call management, patient education, care planning, ensuring patient appointments are made, anticipating patient needs, and working closely with the RN Navigator between systems, locations, and types of therapy offered (Rad/Onc, Surgery, Chemotherapy, Palliative Care, Survival). They will also work closely with the other clinical team members (Physicians, APPs, CMAs, LPNs, RNs, Pharmacists, etc.) and coordinate care appropriately during clinic.
Additional Job Duties:
- Patient care: Responsible for having primary role in the coordination of care for pediatric oncology patients and complex hematology patients (hemophilia, etc). Establish point of contact, review workup schedule, identify barriers and answer questions for new oncology diagnosis. Assists in the development of a patient intake, follow up and triage workflow. Participates in the coordination and planning of patients in the outpatient clinic including new referrals, routine follow-up visits, hospital follow-up visits, and acutely ill patients. Care coordination of referrals and ongoing shared care with external nurse navigators from other pediatric hematology/oncology centers to MUSC. External care coordination and creation of medical record packet for MUSC referrals to external clinical trials, proton therapy (external), rehabilitation facilities, and second opinions. Identify resources available to support patient needs, which includes providing information on community resources and facilitating patent access to basic needs.
- Education: Develop education resources for patients in concert with the supervising physicians to empower patients and caregivers for improved quality of life and adherence to complex medical regimens, including treatment calendars and medication calendars to promote treatment adherence. Education and orientation about clinic guidelines prior to inpatient discharge for new diagnosis oncology patients and BMT patients from outside centers. Serve as a resource to identify potential patients eligible for clinical trials and collaborate with MUSC research staff to offer opportunities to our patients. Help coordinate teaching and home therapy for hemophilia patients and complex bleeding disorder patients. Serve as a resource to clinic nursing and staff on common adverse events in our population and ensure standardized approach to the management of these issues in collaboration with the supervising physicians.
- Coordination of Clinic: Coordination of new patient referrals in collaboration with central and clinic scheduling to ensure efficient access with the appropriate provider is a key responsibility. Review new patient visit planning and scheduling of testing to ensure correct information and appropriate providers are scheduled to see patients. Review schedule and coordination with families for patients having multiple ancillary studies and procedures on same day as clinic visit. Weekly coordination with HCC Clinical Trials office to ensure appropriate labs/tests/assessments scheduled for clinical trial patients. Collaborates with inpatient and outpatient hematology/oncology teams to facilitate patient transitions before and after hospitalization. Communicates with infusion nursing staff, oncology teams, primary care and ED teams, and others as needed to follow up on care plans. Follows up on care plans with a focus on symptom management after clinic visits with phone call or in person visits. Supports patients and families in overcoming barriers to care and facilitates successful clinic visits through appointment reminders, arranging transport and/or use of video visits as appropriate. Medication management including non- chemotherapy medication refills, monitoring for drug toxicity, and prior authorizations will comprise a portion of this time. Quality improvement projects, determined in collaboration with the supervising physician, to ensure the clinic provides standardized efficient care across this population will also be a priority.
- Communication and Outreach: Collaborate with MUSC marketing to enhance outreach through the state and region of our program.
- Participates in the establishment and monitoring of clinical quality indicators and metrics along the continuum of care for pediatric oncology to improve outcomes and measure impact of quality measures. Maintain new oncology diagnosis spreadsheet for tumor registry reporting to HCC and USNWR tracking
Minimum Training and Education: Bachelor's degree in nursing from an accredited school of nursing and three years related nursing experience. Some positions require certification as a generalist in a related specialty area by the American Nurses Association (ANA).
Required Licensure, Certifications, Registrations: Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state.
