What You’ll Do:
- Work in collaboration with the Palliative Care Medical Director and other palliative team members to deliver palliative care by providing diagnosis, treatment and follow-up for patients referred to palliative care.
- Care for patients in their home within a 60 mile radius of our Tigard, OR. office
- Provide regular visits to reassess the patient’s needs, concerns, and update the plan of care
- Initiate communication with the Primary Care physician, other agencies involved in patient care
- Prescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law.
- Order and interpret laboratory and other diagnostic tests within the scope of professional practice.
- Order treatments and durable medical equipment as indicated
Qualifications:
- Licensed as a Nurse Practitioner in the state of practice
- Minimum of Masters of Science in Nursing from an accredited school of nursing, or an equivalent as allowed under state law
- Training/experience in Palliative Care, Gerontology, Geriatric Medicine and/or Chronic Care preferred
- Prior Home Health and/or Hospice experience as an RN or NP preferred
- Practice experience with Part B billing preferred
- Have current DEA license
- ACHPN – Certification in Hospice and Palliative Care Preferred
- Minimum of two years of varied work experience as a professional nurse
- Must possess current CPR Certification
- Demonstrated skill in nursing practices and clinical judgments
- Ability to make home visits and provide nursing care in the home environment
- Ability to cope with family emotional stress and tolerance of individual lifestyles
- Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
- Valid driver’s license and proof of insurance is required
