Overview:
The Mercy HaH Patient Associate License Practical Nurse (LPN) provides day-to-day hospital-based operations and logistics support to the HaH clinical team. The Patient Associate (LPN) is a key member of the HaH operations team and is the 'glue' supporting all hospital-based logistics prior to HaH admission. The LPN is a high-energy, patient-facing team member who coordinates and supports an exceptional patient/family experience during the HaH admission process.
The Patient Associate (LPN) utilizes nursing process to provide patient care under the supervision of the HaH Admitting MD/APP (the clinician responsible for screening, consenting, and admitting all HaH patients). Performs duties and responsibilities in a manner consistent with Mercy HaH mission, values, and the service standard. The Patient Associate (LPN) reports operationally to the HaH Director of Clinical Operations (DCO) who has day-to-day responsibility for HaH operations.
Qualifications:
• Education
- High school diploma or GED equivalent:
- Graduate of accredited nursing program
Licensure:
- Responsible for maintaining current LPN license within hiring state with nursing duties performed and must meet all State Board License requirements
Other:
- Excellent communication skills and detailed oriented approach.
- Ability to self-organize and prioritize work in a fast-paced, matrix environment.
- Computer skills: proficiency with Microsoft Office and Teams; more broadly computer savvy with a demonstrated ability to learn and navigate multiple systems.
- Attentiveness to HIPAA Confidentiality and Privacy Policies.
Preferred Experience:
- 1+ years' experience with patient access and delivery systems, and/or prior healthcare clinical operations experience strongly preferred.
Clinical & Operational Coordination:
• Supports the HaH program operations and logistics as the 'on-the-go' hospital-based resource for all day-to-day HaH activities necessary to support the patient and HaH team prior to the patient’s admission.
• Maintains patient rights and confidentiality of patient information in HaH program.
• Performs HaH assessments as directed by HaH Admitting MD/APP.
• Facilitates and supports Mercy HaH patient screening and program enrollment.
• Coordinates all non-clinical activities based on the “HaH Admission Checklist” including but not limited to the supplies, technology, and equipment required for the patient to be admitted.
• Maintains professional accountability for provision of HaH patient care for assigned patients and coordinates care with other Mercy departments.
• Serves as a liaison under the direction of the HaH Admitting MD/APP to the HaH Hub (virtual command center) and hospital-based services for the Mercy HaH program.
• Coordinates with the HaH Hub to organize transportation services for all admitted patients.
• Assembles the HaH 'Take-Home Kit' and any additional required medical supplies, DME, and/or equipment needs under the direction of the Admitting MD/APP and/or HaH RN.
• Works cooperatively with all hospital staff to ensure that patients and family have a great experience during the HaH admission process.
• Communicates with all HaH team members in a timely manner and responds to requests as they arise.
• Maintains relevant HaH knowledge of Scope of Practice and Delegation of limitations and follows Mercy polices/procedures.
Administrative & Planning:
• Operational support duties include: HaH 'ready-to-go' store inventory management, supply ordering, logistics coordination and facilitation, cleaning/maintaining HaH equipment.
• Attends Mercy HaH staff meetings, daily team huddles, and other meetings as requested.
• Participates in Mercy HaH process improvement workgroups and works continuously toward improvement goals.
• Supports service-related and operational initiatives as they are implemented.
• Miscellaneous tasks include: copying, email communications, US Postal support, faxing, documenting work, and other administrative duties required.
