With over 35 years of service, AID Upstate is the only provider of comprehensive HIV care, prevention, and supportive services in our area, servicing Greenville, Anderson, Pickens and Oconee Counties. We provide support for the whole person, including HIV specialty and primary care, medical case management, referrals/payment for medical care, health insurance premiums and copayment costs, and resources to meet everyday needs of those living with HIV such as our food pantry, transportation, pharmaceutical assistance, mental health initiatives, and much more.
We are currently seeking a bilingual (Spanish/English) Medical Case Manager to join our team. In this role, you will make a meaningful impact by connecting individuals affected by HIV to vital services and helping them navigate both personal and healthcare-related challenges.
Here is some of what you can expect in this role:
- Assist clients in accessing and maintaining benefits programs to ensure they receive essential care and treatment.
- Prepare client histories, action plans, and service reports to ensure comprehensive and client-centered care coordination.
- Work closely with clients through individual or group conferences to help them understand and follow medical recommendations.
- Collaborate with physicians, nurses, and other healthcare providers to create optimal treatment plans.
- Stay up-to-date on available community resources and services to effectively connect clients to what they need.
- Document all client-related activities following AID Upstate and DPH policies.
- Manage a caseload of 100+ individuals, ensuring all client information remains confidential, even after employment.

Requirements:
What you will need:
- A Bachelor's degree in behavioral science or a related field, with at least two years of relevant experience (or equivalent education/experience).
- Fluency, both written and spoken, in English and Spanish is required.
- Proficiency in Microsoft Office (Word, Outlook) and familiarity with office equipment like phones, copiers, and fax machines.
- Strong interpersonal skills and the ability to work with diverse populations.
- Ability to maintain confidentiality and comply with HIPAA regulations both during and after employment.
