Job Overview:
The Community Health Worker (CHW) is an essential member of the Diversity and Inclusion Team. CHWs are frontline team members who have a clear understanding of the communities we serve. This trusting relationship enables them to serve as a liaison between the patient/family and the multidisciplinary healthcare team.
Qualifications:
Required:
- High School or GED required
- Strong understanding of cultural needs related to the Orthodox Jewish community
- Excellent problem-solving skills
- Detail orientated with attention to follow-up
- Effective interpersonal, oral, and written communication skills
- Working knowledge of MS Word and MS Outlook
- Ability to work independently
Preferred:
- Bachelor s Degree preferred
- Moving down in order of pertinence
- Working knowledge of safety event reporting software
Certifications and Licenses Required:
Scheduling Requirements:
- Day shift, 8:00am 12:00pm
- Monday- Friday
- Part Time, 20 hours per week
Essential Functions:
- Linking people to health care/hospital resources; help patients/families in utilizing hospital resources and overcoming barriers to obtaining needed information and services
- Facilitating communication and patient/family empowerment in interactions with health care systems
- Document activities, services, and results in an effective manner while strictly adhering to the policies and procedures in place
- Helping health care team become culturally relevant and responsive to their service population
- Advocating for patient needs; providing informal counseling, support and follow-up
- Work collaboratively and effectively within a team
- Establish positive, supportive relationships with patients/families and provide feedback
- Effectively work with people (staff, clients, doctors, etc) from diverse backgrounds and building and maintaining positive working relationships
- Motivate patients to be active, engaged participants in their health
- Continuously expand knowledge and understanding of resources, services and programs provided; human relations and the procedures used in dealing with the public; operations, functions, policies and procedures associated with the department
