What does a Pinnacle PT do?
As a Home Health Nurse for Pinnacle Home Care, you will travel to a patient’s home to administer services and provide a variety of tasks during patient visits depending on the specific plan of care and type of patient. Some tasks include:
- Complete an initial assessment of patients and determine home care needs including Outcome and Assessment Information Set (OASIS) assessments.
- Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.
- Assists in the development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
- Teaches, supervises, and counsels the family and client in the physical therapy program.
- Supervises Physical Therapist Assistants at least every 30 days.
- Communicates with the physician whenever changes occur regarding the client’s status and obtains additional orders, if necessary.
- Communicates with other team members (including Home Health Aides) and, when appropriate, instructs them in certain phases of physical therapy that they may use while working with the client.
- Writes clinical/progress notes on the date of the visit and incorporates same in the client's record according to company policy.
Qualifications:
- Graduate of an accredited Physical Therapy program or equivalent education.
- Licensed to practice as a PT in the State of Florida.
- Current CPR Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- At least one year of experience as a Physical Therapist.
- Home healthcare experience is preferred but not essential.
- Electronic medical record experience is preferred but not essential.
- Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations. Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
