The Spine and Pain Nurse Navigator is responsible for guiding a patient through the comprehensive spine and pain treatment plan designed for them as a whole person. This role includes education, support, coordination, and advocacy, for the patient and family. The goals of this position include ensuring high quality patient care, easy access to the care needed, barrier elimination, and an excellent patient experience by connecting the patient and family with appropriate resources and services. The Spine and Pain Nurse Navigator will treat all patients entrusted in their care with dignity, showing the love and compassion of Jesus.
- Triage new referrals to ensure they are connected with the correct specialty provider and assist in obtaining their first appointment.
- Provide education to patient and families in language they understand, answer questions, and empower them to take an active role in their Spine & Pain journey.
- Patient advocate
- Facilitator for collaboration with multidisciplinary health team members
- Focuses on an excellent patient experience to meet the needs of the patient while striving for efficiencies in patient care and flow.
- Ensures follow-up with appropriate resources are identified.
- Works closely with providers as the primary patient liaison to support the physician-patient relationship.
- Assists with data collection, analysis and identifying areas of improvement.
Qualifications:
- Experience:2-3 years’ experience required.
- Preferred Experience: 3 years of Spine/Pain
- Required Education: Graduate from approved school of Current RN (Registered Nurse) license in state of practice
- Preferred Education: Bachelor’s Degree
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiac Life Support) required within 90 days of hire/transfer.
- Preferred Certification: Pain Management Nurse Certification
- Preferred Skills: Biopsychosocial assessment skills, time management skills; effectively utilize resources to meet patient needs
- Preferred Other: Public speaking skills, easily understands and integrates new information, experienced with evidence based best practice and protocol development/improvement.
