Job Summary:
Works with interdisciplinary team to develop and provide a program of services and referrals for persons experiencing bereavement.
Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
- Coordinates initial assessment with the development of a plan of care and the provision of identified services.
- Organizes and coordinates grief-related support groups. Trains volunteers to work in program development and directly with the bereaved.
- Develops grief education programs and materials for use by clients and for distribution in community. Serves as a community liaison for bereavement outreach and education.
- Ensures that visits and interventions are documented appropriately according to regulatory standards.
- Works in a constant state of alertness and safe manner.
- Performs other duties as assigned.
EDUCATION
- Master's degree in Social Work or related field
EXPERIENCE
PHYSICAL REQUIREMENTS
- Frequent reaching, sitting, standing and walking.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more).
- Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
- Frequent keyboard use/data entry.
- Occasional bending, stooping, climbing, twisting and gripping.
- Occasional lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Occasional use of vision to identify and distinguish colors.
- Occasional driving.
- Rare repetitive foot/leg and hand/arm movements and squatting.
