Summary:
Coordinates the successful transfer or bed placement of patients entering Memorial Healthcare System (MHS). Ensures patients are assigned rooms in an expedient manner by communicating effectively with the surgery department, emergency department, nursing units, physician offices, and outlying facilities to ensure appropriate transfer and placement of patients. Gives high priority to customer service and complete customer satisfaction.
Responsibilities:
Remotely engages with facility staff to monitor and document transfer need; communicates patient condition to sending or receiving providers and facilities. Consults other disciplines based upon patient s needs, e.g., infection control, equipment needs, nutritional, functional, pain, social and skin integrity.
Serves as the expert in Emergency Medical Treatment and Labor Act (EMTALA) and intervenes including escalation to leadership, as needed. Follows established protocols for interfacility transfer acceptance and escalates to direct report, Administrative Officer, or Administrator On Call (AOC).
Follows established documentation protocols for transfer and bed placement in the Epic EMR to ensure adequate patient safety standards are met.
Collaborates across all disciplines to coordinate patient transfers, including disposition, ambulance transportation (internal and external), specialist referrals, etc. Coordinates the successful transfer and placement of patients across MHS.
Assigns initial bed placement and establishes bed assignments based on documented assessment, patient and bed attributes, isolation and identified risks.
Anticipates admission needs and communicates findings with transferring department, nurse or facility and collaborates to obtain optimal patient flow across the system.
Plans, implements, and evaluates patient needs based on reported assessment to optimize outcomes and maximize available resources. Remotely triages using Critical Care or Emergency Department experience to identify patient care needs providing expert and timely recommendation for correct facility transfer based on patients acuity, services required, condition, circumstances and area facility capacity and capability matrixes.
Utilizes the chain of command to advocate for a patient in need of priority admission or transfer to higher or lower level of care that are beyond the scope of practice for the facility, specialists, etc. (e.g., STEMI, stoke, trauma alert).
Accurately completes effective and expedient placement of patients from all nursing units with emphasis on the ED, ICU, Post Anesthesia Care, Cardiac Care, Cath Lab, and external transfers.
