As a Registered Nurse (RN) Patient Care Manager you will:
- Be responsible for the patient-centered care/services that are provided by the agency, including patient and personnel assignments, coordinating referrals, assuring that patient needs are continually assessed; assuring that agency is developing, implementing, and updating patients’ individualized plan of care
- Be responsible for the agency’s quality assurance and performance (QAPI) program, including patient outcomes and patient experience
- Oversee the implementation and ongoing assessment of the patient’s plan of care through the management of home health RN Case Managers and other personnel
- Create a positive work environment conducive to the attainment of personal and business goals
- Ensure overall compliance with all federal, state, and local government laws and regulations as well as policies and procedures of Interim HealthCare
To qualify for a Registered Nurse (RN) Patient Care Manager position with us, you will need:
- Education: Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required
- Licensure: Current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Home Health Case Management experience required
- One (1) year of supervisory experience as a Registered Nurse (RN) preferred
