Avera is currently seeking a Registered Nurse (RN) – Case Manager looking to deliver the best possible nursing care in Avera’s patient-focused settings.

Job Summary
- The RN Case Manager – Care Transitions manages the care of patients referred to the Care Transitions program.
- Performs patient assessment and develops a patient-centered plan of care including follow-up phone calls and home visits.
- Addresses clinical concerns, provides education, makes referrals to appropriate resources and services, assists with access to care, improves care coordination and assures an effective discharge plan for patients at the end of the Care Transitions service.
- The RN Case Manager – Care Transitions will develop patient care plans in accordance with established protocols. Maintains contact with referring facilities, agencies, and community resources.
Education and/or Experience
- Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.
Licensure/Certification/Registration
- Licensed as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure required.
- Licensed as a Registered Nurse in the State of Minnesota required within 90 days of hire.
Work Schedule
- Day Shifts: occasional weekends, occasional holidays, call involvement is rare; PRN (as needed)
Pay
The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
