Providence is calling a Care Manager LCSW, Behavioral Health who will:
• Support Primary Care teams across the Providence Clinical Network with a variety of administrative and clinical tasks
• Support improvement in depression treatment response across the network
• Work with stakeholders throughout the clinical network to develop protocol and test technology solutions to complete depression monitoring screening tools outside of scheduled visits
We welcome 100% remote work for residents who reside in one of the following States:

Required qualifications for this position include:
• Graduate of a CSWE accredited School of Social Work
• Clinical Independent Social Worker License in the state(s) of hire
• Master's Degree Social Work
• 3+ years professional social work experience
Preferred qualifications for this position include:
• 1+ years of Experience in mental health/behavioral health, counseling, or psychology
