Job Summary:
• Obtains medication histories for new patients and documents them in the electronic medical record in a timely manner.
• Medication history will be obtained from patient, patient's family, pharmacy, physician office, or long term care facility. Employee must be diligent to fully explore the medication history ... • Communicates with pharmacist appropriately including asking questions, and referring for additional interview or intervention.
• Uses good clinical judgement to update the medication history to the best possible list.
• Demonstrates knowledge and skills to appropriately communicate and interact with employees, patients, families and visitors of all age groups while being sensitive to their cultural and religious beliefs.

Qualifications:
Education/Skills:
High school diploma or equivalent required; courses in science and math helpful.
Work Experience:
Retail or hospital pharmacy experience required.
Certification/Licensure:
