Starting Salary Range:$68,656 - $75,000. + Office Benefit Package (Parking/Remote Expenses): $130/Month subject to change
The Mental Health Practitioner Float Lead is responsible working as a float, while also supervising the work of the Float Practitioner team. The float team provide professional, quality program services to the clients served at RADIAS Health. Working across program areas, float practitioners provide direct services including case management services, rehabilitative interventions, and the instruction and monitoring needed to maximize independent functioning. Floats are assigned to work across agency programs to cover time off, leaves and vacancies. Float practitioners work with team members to facilitate continuity of care for program participants. Common assignments include work with our Assertive Community Treatment (ACT) teams, case management, homeless services/supportive housing teams, hospital diversion teams, and residential treatment programs.
It is estimated that approximately 80% of the Float Lead’s time will be spent in direct service across teams, as is customary for the float position. An estimated 20% of time will be dedicated to the supervision and administrative duties associated with supervising the team.
Duties & Responsibilities:
- Provide administrative supervision float practitioners (10-15 individuals) through the employment life cycle, including hiring, assignment of work, provision of routine individual and group supervision, and management of performance concerns.
- As a float, provide direct services to individuals across program areas within the framework, policies and procedure of each program assigned to.
- Track agency coverage needs and creates schedules for float coverage.
- Conduct and document routine individual and group supervision meetings.
- Orient new team members, tracks and develop training plans for team members.
- Coordinate with program managers to facilitate provision of training, feedback and logistics as needed to meet the needs of the service teams.
- Manage PTO requests, approve timecards and partner with Human Resources to manage FMLA and other employee needs.
- Provide coaching, modeling and education to the float team to facilitate their ability to complete quality work within the context of best practices and variable role assignments.
- Float assignments completed by the lead may include providing supervisory coverage to service teams as needed.
- When providing direct services, coordinate care with the manager and team assigned, with a focus on delivering services that support service engagement and continuity. Effectively engaged individuals served, with a focus on respectful, person-centered care.
- Implement the service or treatment plan that has been established by the team. This may include case management, the provision of direct assistance in meeting basic needs, and the delivery of psychiatric rehabilitation interventions.
- Within the context of longer-term assignments, floats may be asked to assume responsibilities for case load management, which may include completion of tasks such as assessments, treatment plans, and court paperwork as needed.
- Assist individuals with accessing resources needed to meet their basic needs, including food, medical benefits, clothing, shelter, harm reduction resources and medical care.
- Support individuals experiencing acute psychiatric symptoms in crisis de-escalation, implementation of coping strategies / crisis plans, and coordination of access to community, residential and hospital-based crisis care.
- Provide education to clients regarding the symptoms, course and treatment of their illness.
- Collaborate with, support and provide education to family members and others interested in the client’s treatment and welfare when warranted.
- Make appropriate / culturally sensitive referrals and assist clients in gaining access to needed services.
- Maintain an awareness of community resources that may be of benefit to individuals served. Provide education and side by side assistance in accessing resources.
- Provide advocacy and bridge communication with community partners, such as other medical providers, landlords, employers, and State and County agencies.
- Will assist clients with making and keeping appointments and any other tasks that are required through the client’s recovery process.
- File all legally required reports with Vulnerable Adults, the Ombudsman and Child Protection.
- Attend and participate in team meetings as scheduled for the team assigned to work with.
- Must be able to transport clients in the community.
- Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
- Participate actively in on-going professional growth and development, maintain appropriate professional behavior, and participate in appropriate supervision.
- Maintain a pattern of regular work hours to meet program needs as determined by the Program Director.
- Complete trainings as required to meet minimum standards associated with work assignments.
- Other duties as assigned.
Supervisory:
- Attend agency meetings and participate in clinically oriented groups as requested by Program Director or Program Manager.
- Meet Minnesota’s Mental Health Practitioner qualifications
- 2,000 hours supervised experience working with individuals with mental illness.
Preferred:
- Masters degree, meeting criteria as a Clinical Trainee
- Experience working in a variety of mental health service settings
- Experience working with a wide range of individuals based on age, race, culture, and diagnosis.
