Who are you?
Telephonically manage caseload of 20-25 cases on a short-term basis for which the primary focus of intervention is care coordination or continuity-of-care.
Candidate may be required to work evening and/or weekend shifts.
Part time hours are 20-29/week.
In this role you will wear many hats, but your knowledge will be essential in the following:
- Utilize multiple sources of information including, but not limited to, paid claims data, customer service call notes, clinical guidelines, medical records, conversations with members, conversations with providers, and plan documents to implement case management care plan as outlined by supervisor.
- Establish and maintain working relationships with internal departments, healthcare providers, client/group, and patients.
- Maintain complete and detailed documentation of care plan progress in Eldorado and UM Web and submit finalized care plan reports reflecting work completed every 4 weeks.
- Ability to meet productivity, quality, and turnaround times on a daily, weekly and monthly requirements.

What you bring to the Personify Health team:
In order to represent the best of what we have to offer you come to us with a multitude of positive attributes including:
- Computer proficiency and working knowledge of Microsoft Office Suite and Microsoft Outlook required.
- Knowledge of medical claims and ICD-10, CPT, HCPCS coding preferred.
- Ability to critically evaluate data and provide accurate patient assessment; ability to establish treatment plans.
- Excellent interpersonal and communication skills; strong customer orientation; good time management skills; highly organized.
- Background in medical terminology
You also take pride in offering the following Core Skills, Competencies, and Characteristics:
- High school diploma or GED.
- 2-3 years prior experience in customer service, claims and/or triage
- Coordination of care experience in an insurance or hospital setting

In compliance with all states and cities that require transparency of pay, the base compensation for this position ranges from $17.00 to $20.00 per hour. Note that compensation may vary based on location, skills, and experience.
