Title: Patient Enrollment Specialist
Role Description
The House Calls Patient Enrollment Specialist (PES) is an integral part of the in-home care team. The PES is responsible for conducting outreach to offer the @HOME program to patients who need our innovative home-based care model. The PES will work closely with teams to continuously identify new and creative ways to connect with patients to engage in the program.
Program Overview
The House Calls Program (known as @HOME) delivers high-quality primary care to Medicare patients in their home. The @HOME program takes a team-based approach to providing outstanding patient care. Our Care Teams - consisting of a Provider, Medical Assistant/Scribe (CIS/MA), Behavioral Health Specialist (BHS) and Mobile Operations Coordinator (MOC) - build individual relationships across a panel of about 75 patients, seen monthly. The Care Team is responsible for delivering excellent, high-touch primary care, and coordinating the care of patients.
Core Responsibilities
The Patient Enrollment Specialist will conduct outbound calls to eligible patients to introduce the @HOME program and encourage patients to participate in the innovative home-based care model.
What we’re looking for
- Track record of success in health-care-related engagement, sales, and/or marketing
- Ability to independently foster relationships with patients and their caregivers
- A passion, patience, and ability to explain complex topics to seniors and their caregivers
- Ability to educate patients about numerous care services accessible through their health plan or their PCP
- A commitment to performing meaningful work that dramatically improves the lives of older adults
- An understanding of the Medicare and Medicare Advantage programs
- A demonstrated ability to operate independently in a fast-paced environment
- Excellent communication, follow-up, teamwork, and problem-solving skills
- Computer skills: Ability to quickly navigate and use multiple computer programs to include, but not limited to: Gmail, MS Word or Google Docs, Excel or Google Sheets, etc.
- Bilingual (English and Spanish) a plus
- US Work Authorization is required

Pay Range
The typical pay range for this role is:
$18.50 - $38.82This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
