Central North Carolina, Post Acute Care Physician Opportunity

Part-time physician to provide in-person coverage one day per week in skilled nursing facilities /post acute
Clinicians trained in or practicing medicine in the following disciplines are encouraged to apply: family, geriatric, primary, internal, adult, acute and post-acute, home health, hospice, palliative and hospital medicine.
Compensation is a productivity-based model (Fee for Service). Expected compensation is estimated range of $72k to $102k annually with no cap on productivity income potential
Provide oversight and mentorship to our clinicians
Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care
Represent group in engaging, establishing, and maintaining relationships within the community
Provide clear communication with clinicians, facility leaders and staff regarding care and services for enrollees
Collaborate with leadership to review care coordination initiatives
Develop intervention plans, and share ideas about network management issues
Work alongside pharmacists on medication management
Complete Medicare required visits
Complete acute, follow up, and discharge visits as necessary
Implement an individualized plan of care for patients' goals
Make recommendations on transitional care, DME, home health services, or outpatient services after a short stay
Facilitate and assist leadership in quality improvement initiatives and risk management
Timely response in chart completions and pharmacy recommendations
