Do not submit candidate declining for other reasons.2 Negative TB skin tests. 1 within 90 days and 1 within 365 days of start (TB Skin tests must state negative results, read in MM, dates placed and read and name/title of HCP administering test) or Quantiferon Gold within 90 days, updated annually.Documentation of 2 doses of varicella 4 weeks apart and/or positive laboratory titerProof of having received vaccine.
