Job Summary:
Manages and maintains, standards, quality levels, cost effective, and customer focused ambulatory social medicine services. Establishes and interfaces with community services. Provides ambulatory social medicine services which add value and are linked to strategic business and organizational goals and objectives.
Essential Responsibilities:
- Plans, develops, implements, and improves/evaluates ambulatory social medicine services, policies and procedures, delivery of social medicine services, quality management, fiscal management, and patient/community planning services.
- Manages staff activities and ensures their competences.
- Develops and/or presents educational/development services that support the delivery of high-quality social medicine services.
- Develops and manages departmental budget and costs including identification and implementation of opportunities for improved services, value enhancement, and cost reduction/management.
- Manages (including collaborating with human resources) and resolves human resources, labor relations, employee performance issues, safety and risk management issues
- Recommends, develops, and implements new programs enhancing quality of care and services to specialized patient populations.
- Identifies community resources.
- Collaborates/consults with physicians, departmental leaders, other departments, other users of ambulatory social medicine to ensure services provided is integrated and comprehensive.
- Analyzes and implements appropriate staffing levels and skill-mix.
- Communicates goals, objectives, accountabilities, priorities, and authority parameters to assigned staff.
Basic Qualifications:
Experience
- Minimum one (1) year of management experience.
- Minimum five (5) years of experience in health care setting.
Education
- Masters degree in social work.
License, Certification, Registration
- Licensed Clinical Social Worker (Hawaii) OR Licensed Social Worker (Hawaii)
- Basic Life Support within 1 months of hire from American Heart Association
Additional Requirements:
- Working knowledge of state and federal health care programs.
- Knowledge of the principles of patient teaching and delegation, care planning, and appropriate utilization of acute hospital, long term care, and home care resources.
- Demonstrated ability to produce and present management reports.
- Demonstrated knowledge of and skill in adaptability, change management, conflict resolution, decision making, customer service, influence, interpersonal relations, oral communication, negotiations, problem solving, results orientation, teamwork, team building, written communication.
