The Senior Health Services of a hospital is seeking a fellowship-trained Geriatric Physician, board-certified in either Internal Medicine or Family Practice, to join a multidisciplinary team focused on comprehensive primary care for seniors. The position involves providing personalized treatment plans based on patients' health concerns and utilizing advanced practices in geriatric medicine. Hospital's Senior Health Services is looking to add a fellowship trained Geriatric Physician BC in either Internal Medicine or Family Practice to multidisciplinary team. The practice has 2 fellowship trained geriatric physicians, 2 Advanced Practice Providers, a full time pharmacist, a social worker, and a community case manager. Practice hours are Monday-Friday, 8 am 5 pm.
The practice offers a full range of primary care for men and women age 60 and older. Our providers take the time to listen to the patient s health concerns and create a treatment plan to meet their health needs. Our physicians have special training in treating seniors and employ the most current information, treatments, medications and practices for disease prevention and diagnosis.
