Job Overview:
We are seeking a dynamic and compassionate Manager of Social Work to oversee and lead a team of Social Workers in a fast-paced healthcare environment in Palm Springs. The Manager will ensure compliance with healthcare regulations, support staff development, and improve patient care through effective discharge planning, psychosocial assessments, and community resource coordination. This role involves clinical supervision, strategic delegation of duties, and collaboration with the interdisciplinary care team to ensure high-quality patient outcomes.
Key Responsibilities:
- Ensure compliance with the Code of Conduct and relevant policies, address concerns, and report suspected violations.
- Supervise and support Social Workers, addressing their needs and concerns in collaboration with the Director of Case Management.
- Delegate and oversee departmental activities, such as scheduling, reviewing 5150 assessments, representing EMC in the community, and organizing educational sessions for the team.
- Provide clinical supervision to MSW staff and support them in achieving licensure requirements.
- Conduct performance evaluations and analyze productivity and effectiveness through monthly reports.
- Complete psychosocial assessments of patients and document thoroughly and objectively.
- Perform 5150 assessments and facilitate patient placement as necessary.
- Develop and coordinate discharge plans with the care team, ensuring timely and efficient execution of discharge arrangements.
- Communicate discharge plans to patients, families, and the healthcare team while respecting patient choice and regulatory requirements.
- Ensure insurance approval for post-acute services and maintain up-to-date knowledge of payer/reimbursement practices.
- Build relationships with community health resources to facilitate safe and effective patient transitions.
- Provide crisis intervention, counseling, and advocacy to support patients and families during hospitalization and treatment.
- Ensure adherence to patient rights, advanced directives, and legal processes, and assist with referrals to appropriate committees or services as needed.
- Serve as a resource to hospital staff on social work-related issues, including APS, CPS, and mental health.
- Promote continuous improvement in clinical processes, ensuring efficiency, cost-effectiveness, and quality care delivery.
- Maintain accurate and up-to-date documentation in accordance with hospital policies.
Qualifications:
- Master s degree in Social Work (MSW) required.
- Current licensure as an LCSW (Licensed Clinical Social Worker) in California.
- Minimum of 5 years of experience in social work, with at least 2 years in a leadership or supervisory role in a healthcare setting.
- Knowledge of hospital social work processes, discharge planning, and compliance requirements.
- Strong leadership, communication, and organizational skills.
- Ability to handle complex cases, including crisis intervention and decision-making.
- Familiarity with community resources and post-acute care services.
- Ability to work collaboratively with an interdisciplinary healthcare team.
- Experience with performance evaluations, staff development, and data analysis.
