Setting: Out-patientHours: Flexible but overall 4-5 days needed, can do Mon-Thursday 8am-5pm CST or Monday-Friday 8am-5pm CSTPatient Diagnosis: anxiety, depression, dual diagnosis, ADHD, mood disorders, SMIMD must be comfortable with prescribing stimulants and other controlled substancesNo on-call needed after hours but will need to be available throughout shift to take refill request for patient panel or when other MDs are outNo supervision of NPsEMR: Epic with Mychart video featureSupport and Wrap around services: Case Manager in PC offices Nurse Triage Team, NP, MAAppointment times: 60-minute initials, 30-minute follow-ups1-hour lunch and 30 minutes of admin time each dayState Licensure needed: Wisconsin and Michigan (will pay for both licensure needed, we could also go through the compact license process)Facility Credentialing timeframe: 60-90 days
