ABout the COMP & BENEFITS:
Excellent Salary
Paid Time Off Twenty (20) working days per calendar year. Coordination with organizational needs and other providers is essential. Carry over is limited to five days per calendar year unless approved by the CEO.
Holidays Nine (9) paid holidays per year include: New Years Day, Good Friday,
Memorial Day, Independence Day, Labor Day, Thanksgiving (2 Days) and Christmas (2 Days).
Paid Dental Insurance, Life, Health Insurance, Vision, and Long Term Disability Ins. Life insurance limited to a cap of
$150,000.
CME:
Allow annual $1,500 to cover costs associated with obtaining CME s, textbooks, magazines and other educational materials. All paid receipts associated to these costs together with proof of attendance must be submitted to CFO before any reimbursement will occur.
Pay for all license and fees associated with employment. New or renewed license must be submitted to CFO before reimbursement will occur.
Malpractice coverage is provided by Federal Tort Coverage in combination with commercial
malpractice insurance coverage.
403- B retirement plan. Eligible for participation after 6 months of employment.
Employer makes quarterly contributions based on salary and employee may contribute in accordance with IRS rules and regulations. Mutual Of America is the company that administers this plan. Each employee initiates and manages their account directly with the Mutual Of America representative. Employer will match up to 2% of employee contribution.
Salary is based upon forty (40) hours per week minimum for five (5) days/week.
Relocation Assistance up to $5000.00.
