A leading healthcare organization is seeking a dedicated and compassionate Advanced Practice Provider (FNP or PA) to join their primary care team on a part-time basis. Based in a juvenile detention facility, this role involves delivering high-quality healthcare to adolescent boys.
Responsibilities: - Conduct comprehensive assessments, including detailed physical examinations and evaluations of psychosocial needs.
- Diagnose, treat, and manage acute and chronic conditions commonly seen in primary care.
- Provide patient education and counseling to promote self-management and improve health outcomes.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Ensure timely access to preventive care and screenings in accordance with healthcare guidelines.
- Serve as a clinical resource to patients, families, and staff members within the facility.
Qualifications: - Active FNP or PA license in the state of New York.
